Job Title: Registered Nurse (RN)
Location: Baton Rouge, LA 70805
Duration: 3 Months+ – (Possible Extension)
Travel: Up to 50% (New Orleans, Lafayette)
Work Schedule: Monday–Friday, 8:00 AM – 5:00 PM (1-hour lunch)
Mileage: Covered from Delmont Village to other centers
Position Overview:
The RN will coordinate patient care, deliver competent nursing services, and provide patient education with a focus on chronic conditions such as diabetes and cardiovascular disease. Nurses will travel between centers as needed to ensure seamless care delivery. The role emphasizes quality outcomes, patient engagement, and collaborative care team support.

Core Responsibilities:
- Deliver competent nursing care consistently and safely.
- Respond to telephonic patient requests promptly and professionally.
- Conduct clinical triage, symptom management, and medication management remotely or in-person.
- Review and update patient medications; manage prescription refills and prior authorizations.
- Educate patients on chronic conditions (diabetes, cardiovascular disease), in person or virtually.
- Collaborate with providers and care team to implement and monitor care plans.
- Build positive relationships with patients and caregivers to support preventative care and reduce ED/hospital visits.
- Delegate tasks to clinical care team members as needed.
- Participate in care team meetings and support clinic operations.
- Perform point-of-care testing, phlebotomy, and other procedures as required.
- Maintain accurate documentation and confidentiality of patient records.
Qualifications / Candidate Requirements:
- Licensure: Active RN license in Louisiana or compact licensure.
- Certifications: BLS from ARC or AHA.
- Experience: 3–5 years of healthcare experience as an RN, preferably in a clinic setting.
