Location/locations: SITE - Rehab & Nursing Kentridge - 4118 Kalamazoo Ave - Grand Rapids
Shift Time: Nights
Hours: 6:30p-7am (1830pm-0700am)
Shift length: 12 hours
Weekend rotation: Every other weekend (Weekends are Sat/Sun)
On Call: None
Required certifications: LPN License, BLS
Scrub Color: Navy Blue

Job Summary: As a continuing care LPN with Client, providing top-notch patient care is paramount. Under the direction of an RN, mid-level Provider, or Physician, you'll be responsible for assessing, planning, implementing, and evaluating patient care. You'll be a vital part of our team, making a real difference in the lives of our patients and their families.

Essential Functions:







ESSENTIAL FUNCTIONS - ROLES AND RESPONSIBILITIES (For this Position)












Collects and monitors assessment data and reports physiological, psychological, sociocultural and spiritual needs of patients and families in coordination with and as delineated by the RN/Mid-level Provider/Physician.









Provides direct patient care under the direction of the RN/Mid-level Provider/Physician, consistent with policies and procedures, and unit based routines.









Informs patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate, in coordination with the RN/Mid-level Provider/Physician.









Documents care provided via computer/written patient record in a manner consistent with guidelines.









Greets and communicates with patients and families in a manner consistent with caring and respect. Communicates and collaborates with other members of the health care team in order to ensure continuity of care and coordination of service.









Cleans patient's environment/work area/equipment and communicates any equipment malfunctions to appropriate department/unit personnel.









Documents patient chargeable items according to standards.









Participates in selected quality monitoring activities under the direction of the RN / Mid-level Provider / Physician. Demonstrates a high standard of moral and ethical behavior. Recognizes appropriate and ethical behaviors and identifies resources for resolution as needed. Advocates for privacy, confidentiality, and security of patient, staff and organization data.
















Actively participates in safety initiatives and risk mitigating measures where appropriate and completes all position and unit safety related competencies and requirements on a timely basis.









Performs other duties as assigned.

  • Client is committed to granting reasonable accommodations in accordance with applicable laws.

  • Once confirmed, all time is submitted through VNDLY by your traveler. Please educate and train them on this. It is also our platform for invoicing and payments so if this step is not complete it will impact your agency's payment from Client.

  • If a Service Provider or Contingent Worker fails to take action required for accurate invoicing within 45 days following the end of a pay cycle in which services were provided, then Client shall not be obligated to make payment to Service Provider for those services.


Examples of action: If staff provided services ending the week Sep 3, 2023 and Service Provider does not submit accurate timesheets no later Oct 18, 2023, Client shall not be obligated to pay for the service provided during that timeframe.



  • Supplier must take system action to implement agreed upon rate changes prior to the week the change becomes effective.

  • It is highly recommended that timesheet reconciliation and issue inquiry/correction is performed by the supplier prior to the 45 days.

  • No other forms of invoicing are accepted outside of our timesheet approval process.
