Job Title: Licensed Practical Nurse - LPN
Location: Grand Rapids, MI 49508
Duration: 3 Months+ (Possible Extension)
Shift: Nights, 6:30 PM – 7:00 AM (12-hour shift)
Weekend Rotation: Every other weekend (Saturday/Sunday)
On-Call: None
Employment Type: Temp/Travel status not specified
Required Certifications:
- LPN License – State of Michigan
- Basic Life Support (BLS) – AHA or ARC
- Driver’s License – State of Michigan
Preferred:
- Graduate of an approved nursing program
Experience:
- Successful completion of competency-based orientation program (Required)
- Healthcare experience (Preferred)
- Specialized experience: Trach care, wound care, EPIC system experience (Preferred/Reliable)
Job Summary:
The LPN provides direct patient care under the supervision of an RN, mid-level provider, or physician. The role focuses on assessing, planning, implementing, and evaluating patient care in a continuing care setting, ensuring patients receive safe, high-quality care.
Essential Responsibilities:
- Collects, monitors, and reports patient assessment data (physiological, psychological, sociocultural, spiritual) in coordination with RN/Provider/Physician.
- Provides direct patient care according to unit policies and routines.
- Educates patients and families on treatments, medications, and equipment.
- Documents care in electronic or written patient records following standards.
- Maintains a clean, safe environment and communicates equipment issues.
- Documents patient chargeable items according to standards.
- Participates in quality monitoring and safety initiatives.
- Maintains ethical standards, patient confidentiality, and privacy.
- Collaborates with healthcare team to ensure continuity of care.
- Performs other duties as assigned.

- Strong communication and interpersonal skills for patient/family interaction.
- Ability to follow established protocols and procedures.
- Ethical judgment and patient advocacy.
- Ability to work independently under supervision.
