Duration: 3 Months+ (Possible Extension)


Shift: Nights, 6:30 PM – 7:00 AM (12-hour shift)
Weekend Rotation: Every other weekend (Saturday/Sunday)
On-Call: None

Required Certifications:

• LPN License – State of Michigan
• Basic Life Support (BLS) – AHA or ARC
• Driver’s License – State of Michigan

 
Preferred:

• Graduate of an approved nursing program

 
Experience:

• Successful completion of competency-based orientation program (Required)
• Healthcare experience (Preferred)
• Specialized experience: Trach care, wound care, EPIC system experience (Preferred/Reliable)

 
Job Summary:
The LPN provides direct patient care under the supervision of an RN, mid-level provider, or physician. The role focuses on assessing, planning, implementing, and evaluating patient care in a continuing care setting, ensuring patients receive safe, high-quality care.
 
Essential Responsibilities:

• Collects, monitors, and reports patient assessment data (physiological, psychological, sociocultural, spiritual) in coordination with RN/Provider/Physician.
• Provides direct patient care according to unit policies and routines.
• Educates patients and families on treatments, medications, and equipment.
• Documents care in electronic or written patient records following standards.
• Maintains a clean, safe environment and communicates equipment issues.
• Documents patient chargeable items according to standards.
• Participates in quality monitoring and safety initiatives.
• Maintains ethical standards, patient confidentiality, and privacy.
• Collaborates with healthcare team to ensure continuity of care.
• Performs other duties as assigned.
