Position Title: Registered Nurse case manager
Location: Albuquerque NM 87107
Duration : 6 Month

This role ensures a seamless patient experience through effective communication, clinical expertise, and collaboration with the care team.

Core Responsibilities
- Provide safe, consistent, and competent nursing care
- Respond promptly to telephonic patient requests, ensuring timely and effective support.
- Deliver clinically sound triage and symptom management for patients both in-person and remotely.
- Follow standardized protocols for medication management, prescription refills, and prior authorizations.
- Perform thorough medication reviews and update records as necessary.
- Educate patients on chronic condition management through in-person, telephonic, or group interactions.
- Collaborate with interdisciplinary teams to develop and monitor care plans across multiple provider panels.
- Build strong patient and caregiver relationships to support preventive care and reduce ER/hospital visits.
- Create a welcoming environment for patients, families, and care team members.
- Delegate tasks to clinical team members to optimize patient care delivery.
- Actively participate in care team meetings to review patient care and clinic operations.
- Perform point-of-care testing, procedures, and specimen collection (including phlebotomy) as needed.
- Ensure an exceptional patient experience through responsive, respectful, and empathetic care.
- Complete additional duties as assigned.
Required Qualifications
- Active Registered Nurse (RN) license in good standing in the applicable state.
- Current BLS Certification.
- Experience with Electronic Medical Records (EMR) systems.
- Strong ability to maintain patient confidentiality and handle sensitive information.
- Excellent communication and assessment skills, including remote assessment capability.
- Minimum 2+ years of RN experience.
- Authorized to work in the U.S.
Strongly Preferred Qualifications
- Proven ability to collaborate within an interdisciplinary care team.
- Proficient computer skills and ability to interpret data from multiple systems.
- Strong problem-solving and prioritization skills.
Preferred Qualifications
- Previous experience in a clinic or primary care setting.
- Ability to work independently in a fast-paced environment.
- Multilingual skills (Spanish, Polish, Russian, or other languages relevant to local communities).
