Job Title: Case Management Coordinator
Job Location: Dupage, Will, Kane, Winnebago and Rock Island County, IL (Remote + Field Visit)
Job Duration: 3 Months+ (Possible Extension)
Shift timings: Standard hours - 8:00 AM to 5:00 PM (Mon-Fri)
Pay rate: $27/hr on W2 (Negotiable)

Position Summary:
- The Case Management Coordinator plays a critical role in delivering comprehensive healthcare management services to members enrolled in Medicare and Medicaid, including dual-eligible populations with complex medical, behavioral, and social needs.
- This role supports the case management process by coordinating care, improving healthcare outcomes, and helping members access benefits, services, and community resources.
- With compassionate communication and strong critical-thinking skills, the Case Management Coordinator collaborates with members, providers, and internal teams to address the full continuum of healthcare and social determinant needs.
Key Responsibilities:
- Collaborate with case managers to support the care management process using sound judgment and critical thinking
- Assist members with appointment scheduling and care coordination
- Identify, educate, and support members in accessing healthcare benefits and available resources
- Utilize care management tools and systems to document and track member interactions
- Support appropriate healthcare outcomes while helping manage healthcare costs
- Communicate effectively with members, providers, and community organizations
- Work independently in a remote environment while collaborating virtually with cross-functional teams
Required Qualifications:
- Bachelor’s degree in Behavioral Health or Human Services.
- (Psychology, Social Work, Nursing, Marriage & Family Therapy, Counseling, or related field).
- OR non-licensed master’s-level clinician.
- Verifiable High School Diploma or GED.
- Minimum 2 years of experience in behavioral health, social services, or a related field.
- Ability to work independently in a remote/home-based environment.
- Strong analytical, problem-solving, and organizational skills.
- Excellent verbal and written communication skills.
- Proficiency in MS Word, Excel, Outlook, PowerPoint, and ability to navigate multiple systems efficiently.
Preferred Qualifications:
- Case management and discharge planning experience.
- Managed Care experience.
- Prior experience working with Medicare/Medicaid or dual-eligible populations.
