Duration: 3 + Months (Possible Extension)
Schedule: Monday – Friday | 8:00 AM – 4:30 PM
Pay Rate: $60 – $65/hr. on W2
Position Overview
The RN Case Manager coordinates patient care management to ensure optimal resource utilization, quality service delivery, and compliance with external review agencies. This role supports patients through comprehensive assessment, care planning, implementation, and evaluation while promoting continuity of care and cost effectiveness.
The RN Case Manager serves as a patient advocate, integrating case management, utilization review, and discharge planning functions to ensure holistic and patient-centered care.

Key Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans
- Coordinate care across interdisciplinary teams (IDT)
- Perform utilization review and ensure compliance with MCG guidelines.
- Lead discharge planning across acute care settings
- Promote cost-effective care while maintaining high-quality outcomes
- Serve as a patient advocate to ensure patient preferences and holistic needs are addressed
- Collaborate closely with physicians, nursing staff, and ancillary departments
- Ensure appropriate documentation and regulatory compliance
Required Qualifications:
- Active California RN License (must be attached to resume)
- AHA BLS Certification (must be attached to submission)
- Minimum 2 years recent RN Case Management experience
- Experience in LTAC or STACH settings
- Strong knowledge of MCG guidelines
- Proven experience with discharge planning across acute care settings
- Strong interdisciplinary team (IDT) collaboration skills
