Location: Lafayette, LA 70503
Duration: 3 Months+ with possibility of extension
Shift: Mon-Fri - 8:00 AM to 4:30 PM

Requirements:
- A state professional registered nurse license or multi-state compact license is required.
- BLS is required.
- Minimum of 1 year experience as a RN in case management department.
- Manages various aspects of the planning process in anticipation of the patient’s movement through the healthcare system.
- Uses care management policies and procedures, including applying evidence-based criteria and levels of care and continuing appropriateness.
- Manages the care of assigned patients through the healthcare system based on the patient’s individual needs.
- Prepares, executes, and reinforces post-discharge care plan.
- Adapts behaviour to the specific patient population, including but not limited to respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions, and communication style.
- Performs other related duties as required.
