Job Title: Family Nurse Practitioner Float
Location: New York, NY
Duration: 6 Months+ (possibility of extension or FT)

Pay range: $74/hr. on W2

Shift: 2-3 days a week and every other weekend.
 
Summary:
We are seeking a dedicated and compassionate Nurse Practitioner to join our clinical care team. In this role, you will collaborate with a multidisciplinary team to deliver high-quality, evidence-based care to a defined patient panel. You will be responsible for assessment, diagnosis, treatment, and ongoing management of patient health, while promoting positive outcomes and patient engagement. This position reports to the Practice Manager.

Key Responsibilities:
  • Deliver evidence-based primary and family care, including assessment, diagnosis, treatment, and health management
  • Develop and implement comprehensive care plans, including health counseling and disposition planning
  • Manage patient appointments and perform routine examinations to support positive health outcomes
  • Order, perform, and interpret diagnostic tests to identify and treat medical conditions
  • Collaborate with physicians and healthcare professionals to ensure coordinated, high-quality care
  • Educate patients on healthy lifestyle choices, preventive care, and chronic disease management
  • Encourage patient participation in their own health and wellness through screenings and behavioral health support
  • Perform administrative duties including clinic opening/closing, inventory management, patient follow-ups, insurance verification, and payment collection
 
Required Qualifications:
  • Master’s Degree from an accredited Family Nurse Practitioner program
  • Current National Board Certification as a Nurse Practitioner
  • Active state license to practice as an Advanced Practice Nurse
  • Strong clinical assessment, diagnostic, and patient management skills
  • Excellent communication (verbal, written, and electronic) and interpersonal skills
  • Ability to work independently and within a collaborative team environment
  • Proficiency with healthcare technology and electronic medical records
 
Preferred Qualifications:
  • At least 1 year of recent experience in primary care or family medicine as a Nurse Practitioner
  • Experience managing chronic disease conditions and quality measures
  • Completion of a primary care training program (if applicable)
