POSITION SUMMARY
Administers direct nursing care to a group of patients under the supervision and direction of a Registered Nurse and assists in the delivery of patient care within the framework of the nursing process. Collaborates with other members of the health care team in meeting the patient(s) identified needs assigned to him/her during a given period of time. The Licensed Practical Nurse is responsible to render service to acutely ill adult patients ranging from 18 years old and older.
DUTIES AND RESPONSIBILITIES
Gather data concerning the patient s physical status through evaluation and communication. Assists in collecting pertinent information for completing admission. Identifies common recurrent patient problems, symptoms, and behavioral changes. Assists RN with discharge, transfer, and orientation of new patients on the unit. Makes sure equipment is available and functioning by testing as per policy. Observes, measures, records, and reports data relating to a patient health status. Performs clinical procedures, such as urinary catherizations, oral or tracheal suctioning, sterile dressing changes, and starting a peripheral IV (with additional training). Reinforce health teaching and counseling to patients and significant other. Guides and assists unlicensed nursing personnel in tasks appropriate to their role, Observes and reports changes in patient s condition to the RN and/or physician (physical, emotional and behavioral changes). Identifies patient goals for consideration by the RN for possible inclusion in a patient s care plan. Administers most types of medications and immunizations and blood (with additional training). Initiates and Occurrence Report for any incidence/situation deviating from the norm. Completes assignment based upon standards of care. Documents according to hospital policy.
Language Skills: Ability to communicate effectively in English, both verbally and in writing. Skills: Basic computer knowledge and word processing skills. Good written and verbal communication skills Ability to use verbal and nonverbal calming techniques to prevent crisis situations. Physical Demands: Involves standing, walking, sitting, lifting and talking. May include some repetitive motions. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position without compromising patient care.
