Job Summary:
The purpose of the Case Manager I position is to support the physician and interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payors. The role integrates and coordinates utilization management, patient education, care facilitation, and discharge planning functions. The Case Manager I is accountable for a designated patient caseload and plans effectively to meet patient needs, manage the length of stay, and promote efficient utilization of resources.
 
Education:

• Bachelors of Science in Nursing, required

 
Required Certification & Licensure:

• Current Michigan Registered Nurse license.

 
Schedule Notes:

• High Priority Request from President of Hospital and Campus Operations.
• Seeking an interim RN Case Manager to be assigned exclusively to the Warren campus.
• This role will report directly to the President and is expected to be primarily onsite.
• Looking for someone with a strong, assertive presence – a “hammer” who can drive the case management process with intensity and precision.
• As described, this person should be an “auditor on steroids,” focused on driving accountability and ensuring process adherence.
• This is not a standard engagement.
