Roles & Responsibilities:
As a Nurse Practitioner specializing in Family Medicine, you will provide comprehensive primary and acute care to patients across the lifespan, including pediatric, adult, and geriatric populations (age range may vary by health center). Working within a collaborative healthcare team, you will deliver high-quality, patient-centered care and contribute to the ongoing health and wellness of the community.

Roles & Responsibilities:
As a Nurse Practitioner specializing in Family Medicine, you will provide comprehensive primary and acute care to patients across the lifespan, including pediatric, adult, and geriatric populations (age range may vary by health center). Working within a collaborative healthcare team, you will deliver high-quality, patient-centered care and contribute to the ongoing health and wellness of the community.
Key Responsibilities:
- Conduct thorough patient assessments, including health histories, physical exams, and evaluation of acute and chronic conditions
- Diagnose and manage common illnesses, injuries, and diseases in a primary care setting
- Order, interpret, and follow up on diagnostic tests such as labs, X-rays, and other imaging
- Develop and implement individualized treatment plans based on evidence-based practice
- Prescribe medications and manage pharmacologic and non-pharmacologic therapies
- Provide health education and counseling on preventive care, nutrition, lifestyle, and chronic disease management
- Perform minor procedures and treatments within the scope of practice
- Coordinate care and refer patients to appropriate specialists or services as needed
- Collaborate with physicians, nurses, and other health professionals to ensure integrated, team-based care
- Document patient encounters accurately and in a timely manner in the electronic health record (EHR)
- Participate in quality improvement initiatives, case reviews, and staff meetings
- Maintain compliance with all clinical guidelines, licensure requirements, and health center policies.
