Duties:
Under the direction of the *** Provider and Clinical Nurse Manager, the *** LVN/LPN is responsible for providing nursing care to participants in accordance with their Product of practice both inside and outside of the clinic setting. Collaborating with the clinic team to facilitate the efficiency of care provided, the LVN/LPN will provide a variety of supports in the clinic and include wound care, POC testing, medication administration and coordination support. The LVN/LPN also will provide a variety of care coordination support tasks outside of the clinic setting within Product of practice and will be a liaison between the participants, their family members, the provider, nurse case manager, and other members of the participants support network, as appropriate. Collaborate effectively with colleagues and stakeholders to promote *** values, team culture and mission. Job Responsibilities: --Clinical:-Perform and document ordered wound care in clinic setting and in accordance with Product of practice, as directed by PCP. -Perform and document ordered procedures and care in clinic, including medication management and administration in accordance with Product of practice, as directed by PCP. -Administer ordered medications to participants while in Center or Clinic and reinforce education for participants regarding medications. -Process Specialty Pharmacy medication orders and escalate to PCP as needed.
-Address participant medication issues and questions within Product. Written plan is needed for education to participants.
-Coordinate with provider on any medication order changes stemming from same-day procedures and manage medication changes as directed by external providers. -Perform medication review/reconciliation during Welbe Welcome Day for new participant visits.
-Perform monthly medication Inventory of ATM Machine in clinic (if applicable). --Care Coordination:
-Perform and document pre and post operative clinical touchpoints, reviewing any instructions provided by the specialist office and direct participant follow-up after procedure, answer any questions within Product of practice, and escalate questions or concerns to the RN Case manager or PCP as appropriate. -Conduct and document high risk monitoring participant touchpoints as indicated for both vitals and/or medication reminders as directed by PCP. -Perform and document proactive quality participant touchpoints for Yellow/Green Risk Stratified participants as indicated by RN Case Manager and escalate questions or concerns to RN Case manager or PCP as appropriate. -Remain alert to pertinent input from other team members, participants, and caregivers and update the RN Case Manager, Provider and/or other clinic staff promptly of any changes in participants condition or medical status. General -Be legally authorized to perform in position location, required.
-Only act within the Product of his or her authority to perform. -Must be willing and have the ability to work a varied schedule that may include evening nights, weekends, and overtime. -Complete all required documentation in a timely and accurate manner.
-Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
-Follow *** policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.
-Communicate regularly with Supervisor and team regarding workload and priorities. -Timely completion of all mandated training and education.
-Timely completion of all mandated occupational health screenings as needed. -Exercises flexibility in performing assignments as business needs evolve. -Other duties as assigned.
Skills:
-Minimum of one (1) year of nursing experience in a clinical setting with a frail or elderly population.
-Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations.
-Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
-Excellent clinical, organizational and communication skills in settings with seniors, their families, and interdisciplinary team members.
-Highly organized to accurately perform numerous tasks for multiple patients.
-Excellent organizational and communication skills.
-Ability to work independently with minimal supervision.
-Demonstrated ability to prioritize in a fast-paced environment.
-Experience and competence working with people from diverse backgrounds and cultures.
-Commitment to unlocking the full potential of our most vulnerable seniors.
