RN assessor (PCA)

Job Summary:
This field-based position is responsible for performing in-home assessments for PCA services using established guidelines and tools to ensure appropriate level of care and services are authorized.

Responsibilities:
  • Assesses for PCA services in the patient’s homes evaluating clinical need aProductst established guidelines and/or standards to ensure that the services provided are medically appropriate and aligned with benefit structure.
  • Meets minimum metric requirement of 5 PCA assessments/day.
  • Evaluates the necessity, appropriateness and efficiency of service provided.
  • Coordinates and assists in implementation of services to meet the needs of the members.
  • Coordinates with patient, family, physician, hospital and other external customers with respect to the appropriateness of care from diagnosis to outcome.
  • Coordinates the delivery of high quality, cost-effective care supported by clinical practice guidelines established by the plan addressing the entire continuum of care.
  • Advocates for the member/family among various sites to coordinate resource utilization and evaluation of services provided.
  • Refers members to MLTSS when appropriate.

    Assignment Details
    Work hours: 40 hours/week with 80% home visits & 20% administrative tasks to update PCA assessments & organize notes.
     
    Shift window: Visits will be between 7:00 AM – 6:00 PM
    Duration:
    Initially 3 months
    Work available for up to 6 months
     
    Equipment provided:
    iPhone
    Tablet
    Other required tech devices for assessments
    Role Overview:
    Homevisit based role (10-20% remote desk job to organize cases & notes)
    Goal is to complete as many inhome PCA assessments as possible
    Responsibilities include:
    Outreach to members
    Visiting members’ homes
    Completing systembased PCA assessments using the PCA tool
    No handson nursing care required
    Limited remote work; majority is inhome visits

    Productivity Expectations:
    5 assessments per day or 20 per week
    Flexibility allowed (e.g. 4 one day, 6 another), but 20 cases/week is mandatory

    Requirements:
    Active RN license
    NJ Driver’s license
    Must have a reliable car
    Mileage reimbursement available (doortodoor)

    Camden County
