Job Title: RN Case Manager
Job Duration: 3+ Months (Possibilities of extension)
Shift: Monday-Friday 8a-5p EST
Location: Columbus, OH 43229
Franklin, Union, Delaware, Pickaway, and Madison Counties for in person visits with 75% travel.
Position Summary
The Case Manager utilizes a collaborative process of assessment, planning, facilitation, and advocacy to coordinate services that meet an individual s benefit plan and/or health needs.
Through communication and available resources, the Case Manager promotes optimal, cost-effective outcomes.
Experience & Skills
Minimum 3 years of clinical practice experience (hospital, home health, ambulatory care, or other alternative care setting).
Experience in healthcare and/or managed care industry.
Case management experience preferred.
Education & Credentials
RN with current, unrestricted state licensure.
Case Management Certification (CCM) preferred.
