Job Title: Registered Nurse (RN) - Case Manager
Location: Morristown, NJ 07960
Duration: 3+ month contract (Possibilities of Extension)
Shift: 8 am – 4 pm Monday - Friday (5*8) hours shift, may require weekends coverage.
Payrate: $57/hour on w2.
Note: Non-taxable Per Diem is provided to the candidate living more than 50 miles from the facility.

Duties:
- Coordinates patients through the acute care hospitalization Manages all aspects of discharge planning/transition process.
- Performs utilization management and quality screenings.
- In- Patient Case Management is required.
- Discharge planning exp required
Requirements:
- 2 years of acute care experience strongly preferred
- Discharge planning, In-Patient Case Management is required
- BSN required
- RN NJ state License or multi state
- CCMC preferred
