Case Manager - Critical Need
Job Title: RN Case Manager
Location: Arlington, VA - 22205
Job Duration: 3 Months (possibility of extension)
Schedule: 8:30AM - 5:00PM | DAY | Every Other Weekend
Job Overview
We’re looking for a detail-oriented and clinically strong RN Case Manager (Patient Care Coordinator) to drive patient outcomes and optimize care transitions for the acute inpatient population. This role sits at the intersection of clinical care and operational efficiency—partnering with physicians, patients, and multidisciplinary teams to ensure safe, timely, and cost-effective discharge planning.
Key Responsibilities
- Conduct initial face-to-face assessments within 24 hours of admission to identify discharge needs
- Develop and communicate patient-centered transition plans with physicians, care teams, and families
- Document assessments and care plans in compliance with organizational and regulatory standards
- Coordinate post-acute care services including:
- Home Health
- Durable Medical Equipment (DME)
- IV Infusion
- Skilled Nursing Facility (SNF) / Sub-Acute Care
- Acute Rehabilitation
- Collaborate with ambulatory case managers and community providers to ensure seamless care transitions
- Support initiatives aimed at reducing readmissions and improving patient outcomes
- Ensure alignment with payer requirements including Medicare, Medicaid, and commercial plans
Qualifications
- Associate Degree in Nursing (ADN) required
- Active RN license (state-specific requirements apply)
- Minimum 1 year of acute care experience (Med/Surg, ED, or Critical Care)
- Experience with Electronic Medical Records (EMR) systems
- Strong understanding of care coordination, utilization review, and reimbursement models
- Completion of PCC Assessment of Critical Skills preferred
