RN - Case Manager
Job Title: RN - Case Manager
Location: Hamilton, Butler, Clermont, or Warren Counties (Ohio) - Remote
Duration: 6 Months+ (With Possibilities of Extension)
Pay Rate: $36/hr. on W2
Shift: Monday – Friday, 8:00 AM – 5:00 PM
Work Setting: Primarily remote/field-based with potential in-person visits depending on assignment area.
Position Summary:
The Case Manager (RN) applies a collaborative process of assessment, planning, facilitation, and advocacy to coordinate health care services that align with the member's benefit plan and clinical needs. This role aims to achieve optimal, cost-effective outcomes through effective communication and use of internal and external resources.
Key Responsibilities:
- Conduct comprehensive clinical assessments of referred members to evaluate needs and eligibility.
- Develop and implement individualized case management plans.
- Interpret and apply clinical criteria, benefit plans, and case management guidelines.
- Evaluate internal/external resources to ensure quality and cost-effective outcomes.
- Engage members, families, and providers in care coordination efforts.
- Monitor, document, and report progress toward care goals in compliance with regulatory and accreditation requirements.
- Collaborate with multidisciplinary teams to support holistic patient care.
Required Qualifications:
- Active, unrestricted RN license in the state of Ohio.
- Proficient computer skills and ability to navigate multiple systems simultaneously.
- Effective written and verbal communication skills.
- Strong organizational, prioritization, and time management abilities.
Preferred Qualifications:
- CCM (Certified Case Manager) certification.
- Previous case management experience.
- Experience in healthcare or managed care settings.
- Clinical experience in hospital, home health, or ambulatory care settings (minimum of 2–3 years preferred).
Work Environment:
- Sedentary role involving extended periods of computer and phone use.
- Occasional field work or site visits may be required.
- Regular inspection and handling of medical and administrative documents.
- Must meet productivity, compliance, and quality standards.
