Education:
Master of Social Work from a school of Social Work accredited by the Council on Social Work Education.
Licensure:
Preferred but not required for this position.
ASW or higher license required, LCSW preferred
Experience Required:
Acute care experience required

Job Description:
- Provide direct clinical social work services, including psychosocial assessments, counseling, case management, and discharge planning for hospital patients and their families.
- Assess and document patients’ emotional, social, financial, environmental, and follow-up care needs in compliance with hospital policies and regulatory standards.
- Develop, implement, and evaluate individualized psychosocial plans of care based on patients’ unique strengths, needs, and health goals.
- Collaborate closely with physicians, nursing staff, and interdisciplinary healthcare teams to support coordinated, patient-centered care.
- Create and manage discharge plans for assigned caseloads, identifying barriers to care and facilitating timely transitions across the continuum of care.
- Monitor patient progress toward established goals and communicate changes or concerns to the care team to optimize length of stay and outcomes.
- Facilitate integration with community-based resources and support services to promote patient recovery, stability, and long-term health.
- Provide counseling and emotional support to patients and families coping with illness, loss, grief, and life transitions.
- Interpret psychosocial, emotional, family, and financial issues for attending physicians and other healthcare professionals to inform treatment planning.
- Advocate for patients and families through an integrated, holistic (mind, body, and spirit) approach to care.
- Complete timely and accurate documentation in the electronic medical record systems (Cerner, MIDAS+) and required departmental forms.
- Participate in quality improvement initiatives, clinical practice teams, research activities, and community networking efforts.
- Demonstrate ongoing professional development and adherence to departmental, hospital, and system policies and procedures.
- Uphold organizational mission, values, and service excellence standards to ensure optimal patient and family satisfaction.
- Perform additional duties as assigned to support departmental and hospital operations.
Core Skills & Competencies
- Strong psychosocial assessment and clinical decision-making skills with the ability to exercise independent judgment.
- Expertise in counseling individuals and families dealing with acute illness, chronic conditions, loss, and grief.
- Solid knowledge of motivational, developmental, systems, and group theories.
- Excellent organizational skills with strong attention to detail and follow-through.
- Effective written and verbal communication skills for interdisciplinary collaboration and patient advocacy.
- Ability to work effectively with diverse populations across the lifespan (neonatal through geriatric).
- Proficiency in electronic documentation systems and basic word processing; database experience preferred.
- Demonstrated commitment to value-based care principles, including Caring, Communication, Collaboration, and Competence.
- Ability to build and maintain productive relationships with internal teams, external providers, and community partners.
- Physical ability to perform job duties, including standing, walking, sitting, and computer use; transportation availability for referrals as needed.
Education:
