Job Location: Fairfield, CA - 94533
Job Duration: 2 Months+ (Possibilities of Extension)
Shift: 8 AM- 4:30 PM. Weekdays and weekends (Days).
Pay range= $55 - $60/hr. – W2

Key Responsibilities:
- Deliver clinical social work services including psychosocial assessments, counseling, case management, and discharge planning.
- Evaluate patients’ emotional, social, financial, and environmental needs while ensuring regulatory compliance.
- Design and implement individualized care plans that support recovery and long-term wellness.
- Partner with physicians, nurses, and interdisciplinary teams to drive coordinated, patient-focused outcomes.
- Lead discharge planning, remove care barriers, and enable smooth transitions across the continuum of care.
- Connect patients with community resources and provide compassionate support through illness, grief, and life transitions.
- Maintain accurate EMR documentation (Cerner, MIDAS+) and contribute to quality improvement initiatives.
- Champion a holistic care model—advocating for the mind, body, and spirit of every patient served.

- Expertise in counseling individuals and families facing acute or chronic conditions.
- Exceptional communication, organization, and relationship-building capabilities.

- Commitment to value-based care through collaboration, communication, and clinical excellence.
Qualifications:
- Master of Social Work (MSW) from a CSWE-accredited program.
- ASW or higher required; LCSW strongly preferred.
- Acute care experience is mandatory
