Job Title: Medical Social Worker (ASW/LCSW/LMSW)
Job Location: Fairfield, CA - 94533
Job Duration: 2 Months+ (Possibilities of Extension)
Shift: 8 AM- 4:30 PM. Weekdays and weekends (Days).
Pay range= $55 - $60/hr. – W2
 
Key Responsibilities:

• Deliver clinical social work services including psychosocial assessments, counseling, case management, and discharge planning.
• Evaluate patients’ emotional, social, financial, and environmental needs while ensuring regulatory compliance.
• Design and implement individualized care plans that support recovery and long-term wellness.
• Partner with physicians, nurses, and interdisciplinary teams to drive coordinated, patient-focused outcomes.
• Lead discharge planning, remove care barriers, and enable smooth transitions across the continuum of care.
• Connect patients with community resources and provide compassionate support through illness, grief, and life transitions.
• Maintain accurate EMR documentation (Cerner, MIDAS+) and contribute to quality improvement initiatives.
• Champion a holistic care model—advocating for the mind, body, and spirit of every patient served.

Qualifications:

• Master of Social Work (MSW) from a CSWE-accredited program.
• ASW or higher required; LCSW strongly preferred.
• Acute care experience is mandatory
