Job Title: Registered Nurse (RN) - Case Manager
Location: Walnut Creek, CA 94598
Contract Length: 3 Months+ (Possible Extension)
Shift: 08:00 AM to 04:30 PM (Day Shift) | Hours per Week: 40
Travel Package: $3150/week
Local Pay: $60 - $70/hr. on W2
(Depending on years of experience)

Key Duties:
- Conduct patient assessments and develop individualized care plans
- Coordinate with physicians, nurses, and interdisciplinary teams
- Monitor patient progress and adjust plans as needed
- Facilitate discharge planning and transitions of care
- Ensure compliance with regulatory and hospital standards
- Provide education and support to patients and families
Requirements:
- Minimum 1 year of experience as an RN Case Manager
- Active CA RN License
- BLS Certification (AHA)
- Strong knowledge of discharge planning, utilization review, and interdisciplinary coordination
- Acute or Hospital Experience
