Summary: Case management services to coordinate patient care, discharge planning, and resource utilization within a hospital setting.
Responsibilities:
- Perform comprehensive case management, utilization review, and discharge planning for acutecare patients.
- Coordinate care transitions, authorizations, and services across multidisciplinary teams.
- Monitor patient progress and ensure appropriate level of care and compliance with hospital policies.
Required Qualifications:
- Active California RN license (unrestricted)
- 1+ year current Hospital Case Management experience
- BLS (AHA)
