Job Title: RN - Case Manager
Job Location: Cincinnati, OH 45211 (Butler, Clermont, Warren, & Hamilton Counties)
Job Duration: 3+ Months (Possibilities of extension)


Pay Rate: $44.11/hr. on W2

Shift: Monday-Friday, 8:00 AM – 5:00 PM EST


Position Summary:
The RN Case Manager plays a critical role in coordinating services for members by utilizing a collaborative process of assessment, planning, facilitation, and advocacy. The goal is to meet the individual’s benefit plan and/or health needs while promoting optimal, cost-effective outcomes through effective communication and resource management.
 
Experience & Skills:

• Minimum of 3 years clinical practice experience in hospital, home health, ambulatory care, or other alternative care settings.
• Experience in healthcare and/or managed care industry preferred.
• Case management experience is preferred.

 
Education & Credentials:

• Registered Nurse (RN) with current, unrestricted state licensure.
• Certified Case Manager (CCM) credential preferred.
