Job Title: Registered Nurse (RN) - Case Manager
Location: Oklahoma City, OK
Duration: 3 Months with possible extension.
Work Schedule: Full-Time | Monday–Friday | 8-hour shifts | 40 hours/week
Job Summary:
- As a vital member of a multidisciplinary care management team, the RN Case Manager is responsible for coordinating the care and services of selected patient populations.
- This role focuses on comprehensive assessment, care planning, resource coordination, and advocacy to ensure optimal outcomes for patients.
Essential Responsibilities:
- Conduct thorough assessments of patient needs, including biophysical, psychosocial, environmental, financial, and discharge planning.
- Monitor patients’ progress for changes that may affect current or future medical treatment.
- Advocate for patients and families to secure necessary services, especially when resources are limited.
- Collaborate with third-party payers to resolve coverage and continuity-of-care issues.
- Lead discharge planning in collaboration with families, the healthcare team, and community resources.
- Review admission data to determine the appropriate level of care using ISD-A or other established criteria.
Education & Licensure:
Required:
- Graduate of an accredited school of nursing.
- Active RN license from the Oklahoma State Board of Nursing or a multistate Compact RN license (eNLC).
- Current Basic Life Support (BLS) certification from the American Heart Association.
Required Skills & Experience:
- Minimum of 3 years of clinical nursing experience in an acute care setting.
- Proficient in nursing practices and procedures with strong clinical assessment abilities.
- In-depth knowledge of community resources for patient support and recovery.
- Familiarity with third-party payer systems, reimbursement, and regulatory requirements.
- Excellent communication skills for conveying complex information to patients and families.
- Strong interpersonal skills to gather patient information and assess care needs effectively.
