Job Description:
- Will this role be fully remote? Yes
- Are there any specific locations the candidates should be in? No, anywhere within the Commonwealth of KY is acceptable as long as they have adequate internet service to support their day-to-day work.
Expected Schedule:
- Typical schedule: 8:30 AM – 5:00 PM EST
- Sometimes members request calls after 5:00 PM. When that happens, the Case Manager can flex their schedule to accommodate (e.g., work 9:30 AM – 6:00 PM) or ask another team member if they can make the after-5:00 call.
Day-to-Day Job Duties:
- This is a clinical role performing care management responsibilities.
- Responsible for a caseload of members.
- Complete health risk assessments and other assessments if warranted based on the member’s situation.
- Complete individual care plans.
- Help members connect with resources.
- Coordinate care with members and providers.
- Ensure all work is completed timely to comply with Medicare requirements.

- Strong engagement skills (ability to connect with people)
- Excellent time management
- Some proficiency with technology and use of applications
Required Education/Certification(s):
- Must be a fully licensed behavioral health professional in the state of KY (LCSW, LMFT, PsyD, LPCC).
- Must have at least a Master’s degree, as these licenses cannot be obtained without that level of education.
Required Years of Experience:
- 5 years of clinical experience
