Location: Toledo, OH 43613
Toledo Region – Fulton, Wood, Ottawa, and Lucas Counties
Job Duration: 3 + Months (Possibilities of extension)

Shift: Monday-Friday 8 am – 5 pm EST

Pay Rate: $40.72/hr. on W2


Position Summary:
The Case Manager is responsible for using a collaborative process of assessment, planning, facilitation, and advocacy to meet each individual’s health needs and benefit plan requirements. This role leverages communication and available resources to promote optimal, cost-effective outcomes for members.
Key Responsibilities:

• Conduct comprehensive assessments of referred members’ needs and eligibility using clinical tools and data review.
• Determine the most effective approach to case resolution by evaluating members’ benefit plans and available internal and external programs/services.

Required Qualifications:

• Registered Nurse (RN) with an active, unrestricted license.
• Minimum 3 years of clinical practice experience in a hospital, home health, or ambulatory care setting.

Preferred Qualifications:

• Case Management experience.
• Healthcare and/or managed care industry experience.
• Case Management Certification (CCM).
