RN Level 2
For Camden County Positions. Majority of assessments are in Camden City
Job Summary:
Responsible for performing RN duties in line with established guidelines to ensure appropriate levels of care and support transitions across the continuum of care. May also mentor or train new RNs and staff as needed.
Key Responsibilities:
- Assess patient needs to ensure appropriate level of care and length of stay.
- Evaluate the necessity and efficiency of medical services and procedures.
- Develop, implement, and coordinate care plans.
- Coordinate services across providers, including out-of-network care when required.
- Monitor patient care and outcomes for effectiveness and appropriateness.
- Advocate for patients and families to ensure optimal care and resource utilization.
- Encourage patient participation in care and disease management programs.
- Maintain accurate and compliant documentation.
- Collaborate with multidisciplinary teams to ensure continuity of care.
- Support quality improvement initiatives and resolve care variances.
- Mentor or train new staff as needed.
- Perform additional duties as assigned.

Qualifications:
- Associates or Bachelors degree in Nursing (or accredited diploma).
- Minimum 2 years of clinical experience.
- 3+ years in the healthcare industry.
- 2+ years of payer (insurance) experience.
Licensure/Certifications:
- Active New Jersey RN license required.
- Preferred: Case Management certification, CDE, or related specialty.
Knowledge & Skills:
- Proficiency in MS Office and standard computer applications.
- Understanding of healthcare systems, payment structures, and care management processes.
- Strong analytical, communication, problem-solving, and organizational skills.
- Ability to collaborate, adapt, and manage time effectively.
