Job Title: RN Case Manager
Job Location: Santa Clara, CA
Job Duration: 3 Months+ (Possibilities of Extension)
Shift: Day shift; 8 AM - 4:30 PM Mon - Fri with weekend rotations every 3 weeks.
Pay Range: $2,720 - $2,840/week on W2
Roles and Responsibilities:

Roles and Responsibilities:
- Demonstrates ability to utilize/apply the general and specialized principles, practices, techniques and methods of Utilization review/management, discharge planning or case management.
- Working knowledge of regulatory requirements and accreditation standards (TJC, Medicare, Medi-Cal, etc.).
- Demonstrates ability to utilize written and verbal communication, interpersonal, critical thinking, and problem-solving skills.
- Demonstrates ability in planning, organizing, conflict resolution and negotiating skills.
- Computer literacy skills required.
Required Qualifications:
- 1 year of experience as a RN Case Manager.
- Current state licensure as Registered Nurse
- Active BLS Required (AHA)
