Shift: Days – 7:00 AM to 3:00 PM or 8:00 AM to 4:00 PM

Role Overview:
LPN/LVN focused on delivering patient-centered care in a long-term care setting. This position emphasizes coordination and completion of MDS assessments, medication administration, and collaborative planning with the interdisciplinary team.

Role Overview:
LPN/LVN focused on delivering patient-centered care in a long-term care setting. This position emphasizes coordination and completion of MDS assessments, medication administration, and collaborative planning with the interdisciplinary team.
Key Responsibilities:
- Coordinate Resident Assessment Instrument (RAI) process and MDS assessments
- Complete comprehensive care plans and attend IDT care plan meetings
- Audit medical records for assessment accuracy
- Facilitate timely transmission of MDS data to QIES ASAP
- Provide nursing care and assist with admissions/discharges as needed
- Maintain compliance with safety, confidentiality, and infection control protocols
- Participate in QAPI, training programs, and emergency preparedness
- Support patient dignity, safety, and rights always
Required Qualifications:
- Active, unencumbered LPN license in Michigan
- Minimum 1 year experience in LTC with MDS completion
- BLS (AHA) certification – Required
- MUST HAVE TRAVEL EXPERIENCE
