Pay Range: $35 - $37/hr. on W2

Department: Care Management / Utilization Review
 
Job Summary:
The Registered Nurse – Care Management plays a critical role in ensuring patients receive the appropriate level of care, coordinated effectively throughout the continuum of healthcare services. This position utilizes clinical expertise to assess medical necessity, manage care transitions, and advocate for high-quality, cost-effective care across multiple settings.
 
Key Responsibilities:
  1. Clinical Assessment & Utilization Review
    • Assess patient clinical needs using established care guidelines.
    • Ensure level of care and length of stay are medically appropriate.
    • Evaluate the efficiency and necessity of medical services and procedures.
  2. Care Coordination & Transition Planning
    • Develop, implement, and coordinate patient-specific care plans.
    • Coordinate services across providers, including out-of-network or out-of-area care.
    • Act as liaison among patients, families, physicians, hospitals, and external partners.
  3. Case Management
    • Support the full continuum of care, from diagnosis to recovery.
    • Monitor care delivery across sites to ensure appropriateness and effectiveness.
    • Encourage patient engagement and compliance with care/disease management programs.
  4. Communication & Documentation
    • Maintain accurate, timely, and comprehensive documentation per guidelines.
    • Interact with multidisciplinary teams via phone or in-person to support continuity of care.
  5. Quality & Fiscal Accountability
    • Understand and apply cost-effective practices without compromising care quality.
    • Participate in problem-solving, variance analysis, and quality improvement initiatives.
  6. Other Duties
    • Perform additional care management functions or projects as assigned by leadership.
 
Qualifications:
Education & Experience
  • Associate's or Bachelor’s degree in Nursing or a health-related field OR graduate of an accredited diploma nursing program.
  • Minimum 2 years of recent clinical experience (acute care, care coordination, or related field preferred).
Licensure
  • Active Registered Nurse (RN) license in the State of New Jersey (Required).
 
Preferred Knowledge & Skills:
  • Familiarity with care management and utilization management principles.
  • Understanding of healthcare contracts and benefit eligibility criteria.
  • Knowledge of hospital systems, payment structures, and case documentation standards.
  • Proficiency in MS Office Suite (Word, Excel, PowerPoint).
  • Experience with Lotus Notes, intranet/internet-based clinical systems (preferred but not required).
