Case Manager I
Jon Title : Case Manager I
Shift: Monday to Friday, 8:00 AM – 5:00 PM MST
Work Type: 100% Remote with required travel for in-home assessments (up to 40%)
Location Requirement: Must reside in New Mexico—specifically in Bernalillo, Sandoval, or Valencia counties
Role Overview:
The Care Coordinator will conduct assessments, create care plans, and support members with medical and/or behavioral health needs. This role involves telephonic outreach, in-person home visits, and collaboration with interdisciplinary teams to ensure continuity and quality of care.
Key Responsibilities:

Conduct clinical assessments within regulated timelines to determine case management eligibility

Develop and implement individualized care plans in collaboration with members and their healthcare support teams

Perform regular follow-ups to monitor care plan effectiveness and make adjustments as needed

Conduct telephonic and face-to-face visits, including in-home assessments

Promote integration of medical, behavioral, and long-term services

Facilitate and participate in interdisciplinary care team (ICT) meetings

Educate and motivate members using motivational interviewing techniques

Identify barriers to care and help coordinate needed services

Consult with RN Case Managers and provide peer support when applicable
Qualifications & Skills:
-
Location: Must reside in Bernalillo, Sandoval, or Valencia counties in New Mexico

Experience:
-
1–3 years in case management, disease management, managed care, or medical/behavioral health setting Or 1+ year of Care Coordination experience with a Bachelor’s Degree

Education:
-
Bachelor’s or Master’s in Social Science, Psychology, Gerontology, Public Health, Social Work, or a related field

Licensure:
-
Must be licensed in New Mexico if licensed (RN not required)

Other Requirements:

Valid driver’s license with a clean driving record

Reliable transportation for up to 40% travel

Strong communication, assessment, and care planning skills
