Shift: Monday to Friday, 8:00 AM – 5:00 PM MST
Work Type: 100% Remote with required travel for in-home assessments (up to 40%)
Location Requirement: Must reside in New Mexico—specifically in Bernalillo, Sandoval, or Valencia counties

The Care Coordinator will conduct assessments, create care plans, and support members with medical and/or behavioral health needs. This role involves telephonic outreach, in-person home visits, and collaboration with interdisciplinary teams to ensure continuity and quality of care.

• Conduct clinical assessments within regulated timelines to determine case management eligibility

• Develop and implement individualized care plans in collaboration with members and their healthcare support teams

• Perform regular follow-ups to monitor care plan effectiveness and make adjustments as needed

• Conduct telephonic and face-to-face visits, including in-home assessments

• Promote integration of medical, behavioral, and long-term services

• Facilitate and participate in interdisciplinary care team (ICT) meetings

• Educate and motivate members using motivational interviewing techniques

• Identify barriers to care and help coordinate needed services

• Consult with RN Case Managers and provide peer support when applicable

• Location: Must reside in Bernalillo, Sandoval, or Valencia counties in New Mexico

• Experience:

  • 1–3 years in case management, disease management, managed care, or medical/behavioral health setting Or 1+ year of Care Coordination experience with a Bachelor’s Degree

• Education:

  • Bachelor’s or Master’s in Social Science, Psychology, Gerontology, Public Health, Social Work, or a related field

• Licensure:

  • Must be licensed in New Mexico if licensed (RN not required)

• Other Requirements:

  • Valid driver’s license with a clean driving record

  • Reliable transportation for up to 40% travel

  • Strong communication, assessment, and care planning skills
