Case Manager I
We're Hiring: Case Manager I | Remote + Field-Based (New Mexico)
Location: Remote (New Mexico Residents Only) + Field Visits
Duration: 6 Months (Possible Extension)
Pay: $48.87/hr
Schedule: Monday–Friday, 8AM–5PM MST
We’re seeking a Case Manager I to join our team and support members across Curry, De Baca, Quay, and Roosevelt counties in New Mexico.
Important Location Requirements:
Applicants must reside in New Mexico, and ideally be based in one of the following counties:
Curry/De Baca/Quay/Roosevelt
This position is hybrid: primarily remote, with up to 40% travel for home visits and in-person assessments. Occasional office attendance for meetings is required.
Key Responsibilities:

Conduct clinical assessments and determine eligibility for case management services.

Develop, implement, and monitor individualized care plans in collaboration with members, caregivers, providers, and support systems.

Facilitate and participate in Interdisciplinary Care Team (ICT) meetings.

Promote behavioral health and long-term services integration.

Travel for in-home assessments and member outreach (up to 40%).
Qualifications:

Education: Bachelor's or Master's Degree (preferred in Social Science, Psychology, Gerontology, Public Health, Social Work, or related field) OR 1 year of Care Coordination experience.

Experience: 1–3 years in case management, disease management, managed care, or related healthcare settings.

License: If applicable, must hold an active NM license. RN is not required.

Other: Valid driver’s license, good driving record, and reliable transportation are required.

Strong communication and documentation abilities

Familiarity with behavioral health and social support systems

Ability to work independently and manage a remote caseload
