RN assessor (PCA)

$40/hr – $42/hr

This field-based role involves conducting in-home assessments for PCA services using established guidelines to ensure appropriate care and service authorization.

• Conduct in-home PCA assessments to evaluate clinical needs
• Ensure services are medically appropriate and aligned with guidelines
• Complete a minimum of 5 assessments per day (20 per week)
• Coordinate care with patients, families, and healthcare providers
• Support implementation of appropriate services
• Maintain accurate documentation and system updates
• Refer members to MLTSS when appropriate
• Advocate for patients and ensure cost-effective, quality care

• Primarily field-based (80% home visits, 20% administrative work)
• Conduct member outreach and home visits
• Complete assessments using system-based PCA tools
• No hands-on nursing care required

• Hours: 40 hours/week
• Visit Window: 7:00 AM – 6:00 PM
• Duration: Initial 3 months (extendable up to 6 months)

• 5 assessments per day or 20 per week (flexible daily distribution)

• Active RN license
• Valid NJ driver’s license
• Reliable vehicle (mileage reimbursed)
