Job Title: Registered Nurse – Home Health
Location: K ne‘ohe, HI
Shift: Day shift (40 hours guaranteed weekly)
Pay Rate: $63/hour
Mileage Reimbursement: $0.43 per mile
Duration: 13 weeks
Role Overview:
The Home Health RN provides high-quality, patient-centered care in the home setting. This role focuses on coordinating and managing care for complex patients, ensuring timely and appropriate services, and acting as a liaison between the patient, interdisciplinary care team, and healthcare providers to optimize outcomes and ensure continuity of care.
Key Responsibilities:

Coordinate and monitor all aspects of patient care across the care team

Ensure care plans are appropriate to the patient's clinical condition and insurance coverage

Collaborate with physicians regarding care plans, diagnostic tests, and discharge needs

Work with case managers on high-risk populations (e.g., chronic pain, substance abuse, homeless)

Facilitate smooth transfers to tertiary care centers, ensuring complete documentation

Review emergency department utilization and manage frequent ED users

Provide oversight for compliance and data collection related to regulatory/accreditation standards

Document care using Homecare Home Base and OASIS

Minimum 2 years of recent Home Health nursing experience

Prior travel nursing experience

Active Hawaii RN license
