Duration: 3 Months+

Shift: Day shift (40 hours guaranteed weekly)

Note: Mileage Reimbursement: $0.43 per mile

Role Overview:
The Home Health RN provides high-quality, patient-centered care in the home setting. This role focuses on coordinating and managing care for complex patients, ensuring timely and appropriate services, and acting as a liaison between the patient, interdisciplinary care team, and healthcare providers to optimize outcomes and ensure continuity of care.
 
Key Responsibilities:

• Coordinate and monitor all aspects of patient care across the care team
• Ensure care plans are appropriate to the patient's clinical condition and insurance coverage
• Collaborate with physicians regarding care plans, diagnostic tests, and discharge needs
• Work with case managers on high-risk populations (e.g., chronic pain, substance abuse, homeless)
• Facilitate smooth transfers to tertiary care centers, ensuring complete documentation
• Review emergency department utilization and manage frequent ED users
• Provide oversight for compliance and data collection related to regulatory/accreditation standards
• Document care using Homecare Home Base and OASIS

 
Required Qualifications & Skills:

• Experience:
  • Minimum 2 years of recent Home Health nursing experience
  • Prior travel nursing experience
• Licensure & Certifications:
  • Active Hawaii RN license
