LPN Case Manager I

Monday-Friday 8a-5p EST
Location: Columbus, OH 43004

Position Summary
The Care Manager Specialist is a member of the Care Team, supporting members enrolled in the Dual Special Needs Plan. This role primarily manages members with low to medium stratification needs or those experiencing social determinants of health challenges. The Care Manager works closely with the Nurse Care Manager, Care Coordinator, Transition of Care (TOC) Coach, and other team members to improve outcomes, coordinate services, and connect members to community resources.
Key Responsibilities
  • Conduct health screenings, assessments, and care planning.
  • Manage a caseload of members with low medium stratification or high social determinant needs.
  • Communicate with members, providers, and care teams to coordinate care.
  • Reduce unable to reach members through telephone and occasional in-person outreach.
Qualifications
  • Education: High School Diploma or GED required.
  • License: Active LPN license required.
  • Experience:
    • Minimum 2 years of LPN nursing experience (3+ years preferred).
    • Familiarity with community resources and services.
    • Experience with care management or telephonic case management preferred.
