New Jersey (Southern Counties – Ocean, Camden, Burlington, Cumberland, Cape May, Gloucester, Salem)
Job Duration: 5 Months+ (With possibilities of Extension)
Pay Rate: $42.27/hr. on W2
Training: 1 Week (Paid)
Travel: 50–75% – Must have reliable transportation. Mileage reimbursed per company policy.
Work Setting: Hybrid (Telephonic and face-to-face assessments in field settings)
Position Summary:
The Care Manager (RN) is responsible for performing comprehensive assessments and ongoing care coordination for members in a variety of settings including private residences, hospitals, behavioral health, and long-term care facilities. This role focuses on building individualized, member-centered care plans and monitoring clinical outcomes to support recovery and well-being.

Key Responsibilities:
- Conduct telephonic and face-to-face assessments of members in homes, hospitals, or facilities.
- Develop and implement cost-effective, personalized care plans in collaboration with members, families, and care teams.
- Monitor and evaluate care plans, adjusting based on clinical judgment and changing member needs.
- Advocate for safe discharges and transitions of care, addressing social determinants of health (e.g., housing, food insecurity).
- Maintain timely and accurate documentation in the electronic health record (EHR).
- Collaborate with providers, care team members, and authorized representatives to ensure service delivery.
- May mentor new hires once proficient in role.
Required Qualifications:
- Current, unrestricted RN license in New Jersey (Must be listed on resume)
- Must reside in the state of New Jersey
- 3+ years of clinical experience with pediatric, special needs, or adult populations
- BSN required
- Reliable transportation for in-person visits (F2F)
- Computer proficiency (EHR systems, MS Word, Excel)
- Strong organizational and communication skills
- Ability to work independently, prioritize, and adapt in a fast-paced environment
- Critical thinking and problem-solving skills
Preferred Qualifications:
- Experience in an integrated care model or case management
- CCM (Certified Case Manager) credential
- Bilingual (preferred)
- Working knowledge of:
- Medical terminology
- Community health resources
- HIPAA and confidentiality standards
- Prior experience with remote/telehealth care coordination
