Position Summary
The Case Manager utilizes a collaborative process of assessment, planning, facilitation, and advocacy to coordinate options and services that meet an individual s benefit plan and/or health needs. - Through effective communication and use of available resources, the Case Manager promotes optimal, cost-effective outcomes.
- Active, unrestricted RN license.
- Clinical practice experience in a hospital, home health, ambulatory care, or other alternative care setting.
- Case management experience.
- Experience in healthcare and/or managed care.
- Case Management Certification (CCM).
