Shift: Monday Friday, 8 AM 5 PM EST
Location: Tampa, FL, 33609

Travel: Up to 75% local travel within Hillsborough, Highlands, Polk, Hardee, and Manatee counties (and neighboring areas)
Position Summary
The Case Manager RN uses a collaborative process of assessment, planning, facilitation, and advocacy to meet members health and benefit needs. This includes supporting care transitions, coordinating services, and promoting optimal, cost-effective outcomes.
Key Responsibilities

• Complete at least 10 nursing facility (NF) member assessments per week
• Support the Health Plan Rebalancing Initiative by assessing, screening, and transitioning NH members into the community
• Conduct in-person visits with current NH members at least twice per week
• Complete home assessments (in-person or telephonic) before discharge to identify environmental support needs (e.g., ramps, DME, modifications)

Requirements

• RN license (Florida) active, unrestricted, and in good standing
• Education: Associate Degree required; BSN preferred
• Experience:
  • Minimum 2 years of clinical experience
  • Managed Care, Home Health, Transition of Care, or Case Management experience preferred
  • Discharge coordination experience highly valued
• Skills:
  • Bilingual (Spanish/English) required
