BHMC - RN Case Manager
Job Title: RN Case Manager
Location: Brooklyn, New York, 11212
Duration: 01/12/2026 – 04/11/2026
Pay Rate: $60-$65/hr.
Schedule: 9:00 AM – 5:30 PM, 5 days per week
Schedule Notes:
• Alternating weekends and holidays
• 37.5 hours per week
• No RTO for new contracts
Required Skills & Experience
-
Minimum three (3) years’ experience in direct patient care.
Education
-
Bachelor’s Degree in Nursing (BSN) – Required.
-
Master’s Degree in Nursing – Preferred.
Licensure & Certifications
-
Current NY State RN License – Required.
-
Basic Life Support (BLS) – Required.
-
Advanced Cardiovascular Life Support (ACLS) – Required.
-
Pediatric Advanced Life Support (PALS) – Preferred.
-
Case Manager Certification (CCM) – Preferred.
Job Summary
-
Coordinates and ensures delivery of care.
-
Collects utilization management and quality assessment data.
-
Interacts with physicians regularly to clarify plans of care.
-
Reviews patients for high-risk criteria for discharge planning needs.
Responsibilities:
-
Monitors patients and determines needs for timely discharge.
-
Confers with 3rd party payers within 24 hours of notification.
-
Conducts initial case review no later than 72 hours after admission.
-
Screens patients for high-risk discharge planning needs.
-
Develops discharge plans for uncomplicated cases.
-
Refers to Social Worker for assessment and discharge planning as indicated.
-
Collaborates with the health care team to facilitate progress of care.
-
Ensures a timely and clearly defined plan of care is developed by the attending physician.
-
Negotiates with physicians and practitioners to develop effective care plans.
-
Monitors duplication of tests and delays in diagnostics, treatment, or scheduling.
-
Ensures documentation of patient progress toward clinical outcomes.
-
Conducts concurrent review.
-
Participates in performance improvement data collection.
-
Reviews cases for continued-stay appropriateness.
-
Collaborates with physician; if unsuccessful, refers to physician advisor.
-
Documents review findings and provides clinical information to 3rd party payers.

Performs retro reviews as requested.

Maintains current knowledge of field developments.

Reviews professional literature and recommends improvements.

Maintains competency through in-service and continuing education.
-
Maintains membership in professional organizations; attends lectures; reads journals.

Develops and maintains high patient and staff satisfaction.

Displays courtesy, tact, and diplomacy with staff, colleagues, superiors, patients, visitors, vendors, and agencies.

Offers assistance to colleagues or secures appropriate help when needed.

Performs other related duties as assigned.
