Job Title: Care Review Clinician II
Shift: Monday–Friday, 8:30 AM – 5:30 PM PST
Duration: 6 months Contract (Temp to hire)
Location: Remote (Candidate must reside in California and hold a valid CA RN license)
Pay Rate: $45.99/hr.on w2
Role Overview:
This position supports discharge planning for a California Health Plan. The RN will collaborate with hospitals, skilled nursing facilities (SNFs), and other lower levels of care to ensure members transition safely and appropriately from acute care settings. This is a remote, temp-to-hire opportunity with an exception for CA-based licensure and residence.
Key Responsibilities:
Coordinate daily with CA hospitals, SNFs, custodial care, LTAC, ARU, and other care facilities for patient discharges.
Ensure members who no longer meet acute care criteria are transitioned efficiently and safely to lower levels of care.
Utilize MCG inpatient criteria to assess discharge needs.
Review and interpret facility contracts related to post-acute care services.
Communicate compassionately with members and providers to support seamless care transitions.
Licensure: Active CA Registered Nurse (RN) license; candidate must reside in California
Required Experience:
Minimum 5 years of clinical experience in acute care (Med-Surg, Telemetry, ICU, or ER).
Minimum 2 years (can overlap) as a hospital-based discharge planner.
At least 3 years of discharge planning experience.
Managed care experience preferred
Skills:
In-depth knowledge of Medi-Cal post-acute care benefits.
Proficient in interpreting SNF and post-acute contract terms
Strong working knowledge of MCG criteriaHigh accountability for patient transition processes
Effective, empathetic communication with patients and care providers.
