- Job Title: RN – Case Manager
- Location: Santa Clara, CA - 95051
- Shift: 8am - 4:30pm
- Contract: 13 weeks
- Pay Range: $60/hr – $75/hr (Stipends Included)
Job Duties
- Conduct comprehensive assessments of patients to develop individualized care plans.
- Coordinate patient care across interdisciplinary teams, ensuring smooth transitions between levels of care.
- Monitor patient progress and adjust care plans as needed to optimize outcomes.
- Provide patient and family education regarding treatment options, discharge planning, and community resources.
- Facilitate utilization management by evaluating medical necessity and appropriateness of services.
- Collaborate with physicians, social workers, and allied health professionals to support patient-centered care.
- Document all case management activities accurately in the EMR system.
- Support discharge planning to reduce hospital readmissions and improve continuity of care.
Requirements
- Active California RN license in good standing.
- Minimum 2 years of clinical RN experience, preferably in case management, utilization review, or discharge planning.
- Prior experience in acute care or hospital setting strongly preferred.
- Proficiency with electronic medical records (Epic preferred).
- Current BLS certification (AHA).
