Shift/Schedule: Monday - Friday, 9:00 AM - 5:30 PM
Pay range: $3000/Week = W2: $41.20/Hour || Weekly Per Diem: $1350/Week

Job Description:
- Coordinate management of care to ensure optimum utilization of resources, service delivery, and compliance with external review agencies.
- Provide ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs.
- Enhance the quality of patient management and satisfaction to promote continuity of care and cost-effectiveness through the integration of functions of case management, utilization review and management, and discharge planning.
- Serve as a patient advocate through resource utilization, discharge planning, and addressing the holistic needs of the patient.
- Must have BLS certification through the American Heart Association
- Must have 2 years of recent RN Case Management and discharge planning experience in a hospital setting.
- Must have a California RN license
