Registered Nurse (RN) – Case Management
Job Title: Registered Nurse (RN) – Case Management
Location: Slidell, LA
Duration: 3+ months (Possible extension or FT Conversion)
Shift: Days/ Nights - 3X12
Job Summary:
The Registered Nurse (Case Manager) functions as a clinical leader within the framework of the Nurse Practice Act and ANA Code of Ethics. This role is responsible for coordinating patient-centered care across the continuum, utilizing clinical expertise, discharge planning, and interdisciplinary collaboration. The RN demonstrates accountability, cultural sensitivity, and commitment to quality outcomes in alignment with the organization’s strategic goals and nursing philosophy.
Qualifications & Skills:
Education:
- Required: Registered Nurse Diploma
- Preferred: Associate’s or Bachelor’s degree in Nursing
Licensure & Certifications:

Required:
- Active RN license in the state of practice
- Basic Life Support (BLS) – American Heart Association

Preferred:
- Certification in Case Management (CCM) or Certified Professional in Healthcare Management (CPHM)
Experience:
- Required: Minimum of 2 years of related case management or clinical nursing experience
Skills & Competencies:
- Proficient in EMR systems, computers, and web-based tools
- Strong verbal and written communication skills for interdisciplinary coordination
- Excellent time management and organization skills
- Effective conflict resolution and sound clinical judgment
Job Responsibilities:
- Coordinate various components of the discharge planning process, ensuring seamless patient transitions through the healthcare system
- Apply evidence-based care management practices, including level-of-care determinations and appropriateness reviews
- Assess patient needs and collaborate with providers and social workers to develop individualized care plans
- Execute, educate, and reinforce post-discharge instructions and follow-up care protocols
- Adapt communication and behavior to the specific cultural, linguistic, and emotional needs of each patient
- Monitor and document patient progress, referrals, and transitions across care levels
- Ensure compliance with regulatory requirements and organizational policies regarding privacy, ethics, and patient safety
- Participate in quality improvement initiatives, staff meetings, and continuing education as required
- Maintain up-to-date knowledge of federal, state, and local case management laws and standards
Physical & Environmental Demands:
- Medium Work: Requires exerting 20–50 lbs occasionally, 10–25 lbs frequently, or greater than negligible up to 10 lbs constantly
- May involve sitting, standing, walking, and use of arm/leg controls
- Potential exposure to communicable diseases, hazardous waste, or medications in patient-care environments
- Use of PPE and adherence to infection control protocols required in certain clinical situations
