Job Title: Registered Nurse (RN) - Care Coordinator
Location: Minneapolis, MN 55455
Duration: 3 months+ with possible extension
Schedule Notes: 8:00 am – 4:30 pm
Local Pay: $50/hr. on W2
Travel Payrate:
Overall Weekly Pay Rate: $2,229.85
W2 hourly Pay Rate: $28
Weekly Stipend: $1,109.85

Requirements:
- Graduate of an accredited or NLN-approved RN program – Required
- A state professional registered nurse license or multi-state compact license is required.
- A minimum of 1 Year of Experience – Required
Job Summary:
- The Care Coordinator RN provides a deliberate patient and family centered partnership with the healthcare team that facilitates a seamless patient experience between the Inpatient and Outpatient settings.
- The Care Coordinator works to maximize the patient’s health outcomes and the patient experience by identifying patient and family needs, completing pre-visit planning, supporting the clinic visit and providing closure to the clinic visits.
- Works to develop and support a coordinated plan of care between visits to promote a smooth transition for patients and family across the healthcare continuum that instills a sense of trust and patient advocacy.
