Job Title: RN Home Care
Location: Manhattan , Long Island, Suffolk, Nassau, Brooklyn, Upstate NY and Bronx open you can choose as per your preference.  
Duration : 6 months with maximum extension possibility  
 
Shift Time: 8 AM – 4:30 PM EST
 
Schedule: HYRBID JOB, 3 days on site , 2 days’ Work from home, Mon – Fri + 1 sat every month
Pay Range: $50.30/hr on W2.
 
Also, If you refer someone who ends up being hired, you may be eligible for a referral bonus.
 
Languages:
Spanish or Russian, or Haitian – Creole or Chinese, or Korean
 
Patient Population :
Psychiatric peds population
 
Reimbursements: Travel reimbursement provided , gas, mobile, etc.  
 
Schedule Notes:

• 8 AM – 5 PM EST Hybrid
• Current valid unrestricted NY State RN license.
• Minimum of 2 years of home care experience in past 2 years is required.
• Experience working with children with Psychiatric issues.

 
Role & Responsibilities:

• Working in the community conducting strength-based assessments for families who have children (ages 0-21) with behavioral health/psychiatric issues.
• All nurses are provided support and education for ongoing professional development and annual certification in the CANS-NY assessment tool.
• The assessments will be the basis for family-driven Plan of Care (POC) development to support the family’s access to Home and Community Based Services (HCBS).
• The Nurse Evaluator will conduct 1-2 assessments a day and work with a Family Support Coordinator to meet the family’s case management needs specifically related to the HCBS POC.
• This position is field based with some initial training provided in Manhattan area.

 
License and Certifications:

• NY State RN License
• NY State Driving license required

 
Preferred Experience:

• Home care experience using the UAS-CANS-NY system or like the UAS such as OASIS.
• Experience in assessments that are conducted face to face, telephonically or telehealth.
• RN experience in homecare/psychiatric/behavioral health/pediatrics/adolescent/foster care settings/group home/school nurse/care management.
• Consistent technological experience using computer systems for assessments/word processing tasks.
• Experience in developing Plans of Care.
