Position Summary:
- The Field Case Manager will support elderly individuals and those with disabilities who are approved for in-home or nursing facility care.
- This role involves conducting in-person member visits across assigned regions and working remotely in between.
- Must reside in one of the following IL areas: Chicago Heights, South Chicago Heights, Lynwood, Ford Heights, Sauk Village, Matteson, Olympia Fields, Park Forest, or Richton Park
- Active, unrestricted IL license as one of the following:
- Registered Nurse (RN)
- Licensed Clinical Social Worker (LCSW)
- Licensed Clinical Professional Counselor (LCPC)
- Valid driver s license and reliable vehicle required.
- Must live near or within assigned travel areas (South Side Chicago / Suburban Cook
Preferred Experience:
- 3 5 years of clinical practice experience.
- 2 3 years in case management, discharge planning, or home health coordination.
