Duration: 3 Months+ (Possible for Extension)
Shift: Monday–Friday, 8:00 a.m. – 5:00 p.m. EST
Pay Rate: $40.72/hr. on W2

Location Requirement:
- Candidates must list their county at the top of their resume.
- Looking in Cincinnati Region: Hamilton, Butler, Warren, Clinton, and Clermont Counties.
Position Summary:
- The Case Manager utilizes a collaborative process of assessment, planning, facilitation, and advocacy to coordinate services that meet an individual’s benefit plan and/or health needs.
- Through communication and available resources, the Case Manager promotes optimal, cost-effective outcomes.
Experience & Skills:
- Minimum 3 years of clinical practice experience (hospital, home health, ambulatory care, or other alternative care setting).
- Experience in healthcare and/or managed care industry.
- Case management experience preferred.
Education & Credentials:
- RN with current, unrestricted state licensure.
- Case Management Certification (CCM) preferred.
