Job Title: Registered Nurse (RN) - Case Manager
Location: Palmdale, CA
Duration: 3 Months+ (Possible Extension)

Schedule: 8:30 AM – 5:00 PM or 9:00 AM – 5:30 PM, One weekend shift per month required
Pay Rate: $80/hr. on W2 (Travel)
**Available Departments: Case Manager

Note: Stipend is also provided to the candidate living more than 50 miles from the facility.
Referral bonus on each successful referral.

Role Overview:

• The Case Management RN is responsible for coordinating and facilitating comprehensive patient care through assessment, planning, implementation, and evaluation.

 
Qualifications & Skills:

• License: Active CA RN License (required)
• Certifications: BLS (required)
• Education: BSN preferred; Associate degree acceptable with relevant experience or BSN enrolment

 
Experience:

• Minimum 2 years RN experience required
• At least 2 years in Case management within a hospital setting (required)
• EMR: EPIC experience highly preferred
