The Registered Nurse – Extended Care provides comprehensive, patient-centered care focusing on the total individual, including physical, psychological, cultural, and social needs. The RN utilizes the nursing process to assess, plan, implement, and evaluate care while acting as a resident advocate and ensuring high-quality outcomes.

Position Overview
The Registered Nurse – Extended Care provides comprehensive, patient-centered care focusing on the total individual, including physical, psychological, cultural, and social needs. The RN utilizes the nursing process to assess, plan, implement, and evaluate care while acting as a resident advocate and ensuring high-quality outcomes.
Key Responsibilities
Patient Care & Assessment
- Perform holistic assessments and develop individualized care plans
- Monitor and evaluate patient/resident conditions continuously
- Incorporate diagnostic data (labs, imaging) into care planning
- Identify risks related to nutrition, function, and safety
- Collaborate with physicians and interdisciplinary teams
- Set priorities and adjust care plans based on patient needs
- Delegate tasks appropriately to care team members
- Administer medications and IV therapies per policy
- Manage urgent/emergent care situations
- Ensure compliance with CMS and Medicare documentation standards
- Educate patients and families on care plans and recovery
- Coordinate discharge planning and follow-up care
- Document patient education and outcomes
- Supervise and support CNA staff
- Act as a role model and maintain professional standards
- Participate in quality improvement initiatives
Required Qualifications
- Active Registered Nurse (RN) License (state of practice)
- BLS – American Heart Association (AHA)
- Associate Degree in Nursing (ASN)
- Ability to meet physical demands (lifting, standing, patient handling)
Work Environment & Physical Requirements
- Frequent standing, walking, lifting (up to 100 lbs)
- Patient handling and mobility assistance required
- Strong documentation and communication skills
