Nurse Case Manager II

Job Title: RN Case Manager Rebalancing Program (Field-Based)
Location: Orlando, FL 32789
Shift Timing: Monday Friday, 8:00 AM 5:00 PM EST
Duration: 5+ Months
Travel: Local travel up to 75% within service regions and neighboring counties

Position Summary:

• The RN Case Manager will support the Rebalancing Initiative by assessing nursing facility (NF) members for potential transition to home or community settings.
• This field-based role uses clinical judgment and evidence-based criteria to ensure safe, cost-effective transitions that enhance member satisfaction and align with contractual goals.

Key Responsibilities:

• Complete at least 10 nursing facility member assessments per week Conduct in-person visits to nursing facilities at least twice weekly to complete rebalancing events and screening assessments
• Assess member homes (via phone or in-person) prior to discharge to determine necessary environmental supports (e.g., ramps, DME)

Required Qualifications:

• Active, unrestricted RN license in the state of Florida
• Associate Degree in Nursing (ADN) required; BSN preferred
• Must reside in the Orlando area
• Bilingual (Spanish/English) required

Preferred Experience:

• Managed care and case management
• Home health, discharge planning, or transition of care coordination
• Working in a fast-paced, dynamic environment
